3. Select Coronavirus or Health Problem

4. Complete and submit patient questionnaire form to the doctor

5. Contact the doctor’s office to confirm the tele visit appointment time.

6. Doctor will call and provide the consultation by phone. If required doctor may send e-prescription directly to your preferred pharmacy.
