2. Verify and update your primary care physician details & submit

3. Annual Exam form will be sent to your doctor

4. Your doctor can access and complete the form based on your annual exam and test results

5. You will receive a text/email alert when your reports are ready and you can access 1) Annual Exam Form, 2) Internal Health Report/Score, 3) Lab Results, and 4) EKG Report through your app
